1. Navigate to Config Section
In Step 2 of the review cycle creation, click on the "Config" section

2. Select form
Select the form for which you want to have custom attributes and turn on the toggle button

3. Save And Continue
After selecting the form. Click “Save & Continue” to proceed to Step 3, where you can create custom attributes

4. Click Dropdown for Custom Attribute Creation
Under Advanced Options (Custom Attributes), click the dropdown next to it to create a custom attribute

5. Enter Custom Attribute Name
Enter the attribute name in the text box that you want to create. After typing the custom attribute name in the text box, the attribute name will appear in the dropdown list

6. Create Custom Attribute
Click the custom attribute name from the dropdown list to create it. The custom attribute will then be added

7. Delete a Custom Attribute
If you want to delete a custom attribute you created, click the “×” icon next to the attribute name. This will remove the custom attribute you no longer need

8. Delete Attribute
Click "Delete" to remove the selected custom attribute

9. Edit Form
After creating the custom attribute, click on "Edit Form" under the Self Review

10. Select Question Block
Click on the “Question” block to add questions to the form

11. Select Custom Attribute
Select the attribute for which this question should be added

12. Click Add Question Block
Click “Add Question Block” to add this question to the form

13. Create Another Block
If you want to create another question, click on "Create Another Block" to add an additional question block

14. Click on Dropdown Icon
To verify which questions have been added to each custom attribute, click the dropdown next to the custom attribute name

15. View Questions Assigned to a Custom Attribute
After opening the dropdown, you will see a list of all created custom attributes. Select the attribute you want to view to check which questions have been added to it

16. Save Form Changes
After adding all the questions to the custom attributes within the form, click “Save & Continue” to save the changes and proceed

17. Finalize Changes
Click Save & Continue to move to Step 4: Reviewees, where you need to assign the custom attributes to the reviewees

18. Click the Balance Scale Icon
Click on the “balance scale” icon present in green color next to the reviewee’s name for whom you need to assign the custom attribute

19. Click on Edit
In this step, you need to assign custom attributes to the reviewee by clicking “Edit.” If there are only few reviewees, you can assign the attributes directly from this step

20. Select the Attribute
Choose the custom attribute you want to assign to the reviewee

21. Click Save.
Click “Save” to assign the custom attribute

22. Import CSV File
If there are many reviewees in the review cycle, you can click “Import CSV” to assign custom attributes in bulk

23. Download Template
Click Download to get the CSV template for importing data. Please make sure not to change the template format use it exactly as provided. In the sheet, you will see a column named “Custom Attribute.” Enter the name of the custom attribute in this column, in front of the reviewee’s name, that you want to assign

24. Upload CSV
After assigning the custom attributes in the CSV, upload it back by clicking “Click here to Upload CSV.” This will assign the custom attributes to multiple reviewees in bulk through the CSV
